In an effort to reduce burden and increase efficiency, effective January 1, 2023, the Centers for Medicare & Medicaid Services (CMS) will be discontinuing the use of two forms. The certificate of medical necessity and the durable medical equipment information forms are currently required to help document medical necessity. However, feedback from suppliers and treating physicians suggests that these forms are duplicative of information already documented in the patient record and in claims.
